Put the new clutch parts in exactly the same way the old one comes out. Just pay attention when you take all the parts out of the basket.

If the relay is good and fan motor is good, outside of a bad connection theres only one thing left and that would be the sensor sending unit just off the thermostat. I never had one go bad though.....

Call your ford dealer, I am pretty sure there is a factory recall on the very thing you mentioned, they should fix it for free. They did fix my 97 model, took about 2 hours and no charge.
