We thought our dog was potty trained and now she seems to be acting out help?

With potty pads you've taught her that eliminating inside the house is okay, and that a soft absorbent surface is the best place to go -- this includes both potty pads AND your carpets. Stop doing this immediately. Look up information on crate training (Google is an excellent resource) and begin training her to go OUTSIDE.
 
We are using potty pads and she was using them just fine we didn't even have to watch her for a couple of months and now she won't even go to her pad she just pees on the carpet whats happening and what can we do to fix it.
