Outlook sending messages from other people's email addresses?

I'm using Microsoft Exchange with Outlook Web Access version 14.1.218.0.

Sometimes when you forward an email it arrives with the wrong From address. Instead of arriving with the From address of the person who forwarded the email it arrives with the address that was on the email being forwarded. Which is causing a lot of confusion.

For example.

If you receive an email from [email protected] and then forward it to somebody else using Outlook Web Access the email that you send says that it's from [email protected], rather than from your email address. So if you reply to it, it goes to the original sender, and not the person who forwarded the email to you.

Does anybody know why this is happening, or how I can stop it? It only seems to effect a handful of emails, so I don't know if this is a technical fault, a setting on the Outlook client, or something that people are doing wrong by accident when they forward an email.

I don't know if it's relevant, but some of the emails have "mailto" in their address string.

This isn't a virus. All of the emails are legitimate.

This is effecting multiple users and seems to be more or less random. I've not been able to replicate it under controlled conditions, but I've seen example of it after it's happened.
