    Engine misfire of some sorts. That's bad. You could ruin your $800 catalytic converter if you keep driving it like that. Have you checked for loose spark plug wires??

    It's probably a bad/worn fuel pump. Yes, the one in the gas tank. Especialy if you have run out of gas, or run very low on fuel in the tank. The fuel is what lubricates the pump and cools it. If the pump ran dry, it's "toast". It's not pumping gas fast enough, or it's not holding fuel line...

    Use a wrench, or ratchet to pull, or swivel the tensioner away from the belt. (against the spring tension) Oh, don't loosen the mounting bolt that holds the tentioner to the engine.     Sure. Find a complete rear axle with disc brakes in a junk yard and replace the whole thing. That's probably the easiest and least expensive way to go.

    Most batteries are made in the same place, they just stick different stickers on them. It could be bad just by the "luck of the draw".

    The engine is hot, the heater core is not. Coolant is not flowing through the heater core. Toyota uses a flow valve to allow the coolant to flow through it. The valve is probably stuck. Follow the heater hoses until you find the valve and see if it's stuck. Maybe even replace it, or back flush...
