Did you hook everything up the way your Yamaha Owners Manual advised? I think there may be operator error involved here.

I would look for loose converter bolts (as already suggested) and for a cracked flex plate (to which the converter is bolted). I have seen two or three of those in 20 years, so it's rare but possible.
