My practicum student asked me to write a letter of recommendation for her portfolio. The problem is, I've never written one before and she wasn't a great practicum student. I'm not sure how good of a teacher she will be either... She didn't do anything wrong per se, just lacked the ability to jump in and be proactive with the kids. Rather lackluster all around and didn't seem too interested in the field... How can I write a letter that is accurate but not offensive to her? She's a nice person and kind, well prepared w/lessons and such... I'm just at a loss!

Just focus on the positives, really focus. Give specific examples. Are you the only teacher writing a letter of recommendation? In which case you will have to ask her what achievements ( that are measurable ) she has in areas you may not know about.

If you don't think you can write a letter that is complementary enough, perhaps you need to explain kindly that you want her to succeed and you don't think you know enough about her from your class to write a good letter of recommendation. Is there anyone else who might be more helpful? If you know the answer is no then unfortunately this reply won't be very helpful, but don't ask if you know the answer.
