Will I change my body frame until April if I follow this exercise and meal plan ?

Here is the meal plan :
- breakfast : muesli with skim milk
- snack : 1 sandwich ( whole grain bread ! ) + veggies + ham
- meal : lean meat with a lot of veggies
- snack : fruits
- dinner : shrimps or veggies or nothing ( it depends )

Exercise plan : the exercise plan is for EVERY day and it's 1 hour Pilates+ 1 hour aerobic
My goal is to lose weight and tone up and NOT only to maintain my current weight. Will this meal and exercise plan show some effect until April ?
Oh, and it's a good idea to eat bread ? I would only eat it once a day as you could see ( at the first snack ), but if I can get good results by not eating any bread then I'll do it ! Just tell me what to do !!!
