Will I be able to get into college?

I tend to panic about my future, so I'll try to explain clearly: I'm a home schooled sophomore, and I'm getting a high school diploma through an umbrella school. I currently have 6 credits (from my freshman year) and this year I'm earning 7.5. I'm very involved in my church, I volunteer with adults with special needs weekly, I sing in choirs that rehearse 2-3 times a week, I take private violin and voice lessons, and this year I've earned almost 200 community service hours. I'm fairly good at singing and violin, I plan to major in music. During the summer I volunteer, go on choir tours (last year we performed in 3 countries in Europe), and attend summer programs. I plan on taking some Community College or AP classes next year, and continuing with all the activities listed above. When I attended school, I was an all A student and I still get those grades (but grading is less important when you're home schooled). I haven't taken the SAT yet, but I got a 1230 out of 1600 on a practice test for it.
My dream schools are Smith College and Case Western Reserve. Am I on track for a high school student planning to attend one of these schools? What should I do to improve my chances of getting in? I've done summer programs at Smith so I already know several professors and I'm familiar with the school.
