Why is my php upload form not working?

I am on Yahoo Small Business server and I have written a couple of upload forms in php/html/mySQL where the user can select the photo or music snippet and upload it. Everything seems to work well except that the resulting files are unreadable. The title data and ID are stored perfectly in the mySQL table. I have a tmp file in the corresponding directories. I have tested the forms with a simple, tiny "txt" file with the same results as well as with both image files and small music files (all under 100k, some files as little as 3k).

I have searched the web for answers.... used three different techniques and believe the issue has to be with the server setting somewhere.

So, what might I be doing wrong?
