Why is my 98 Honda Accord overheating?

Yes it's old but it runs really well. I get the oil changed every 3,000 miles, had a new tranny put in at 100,000 miles (it now has 150,000). The air conditioner usually works well, except for being extremely loud for the past several years (reasons why?).
Recently my car was overheating. I had it checked and the radiator cap was broken and and it was empty. Got a new cap, refilled it and it's been fine. Well, the other day I was driving in approx 103 degree weather for about 1/2 hour and the a/c started blowing out warm air and the car got almost all the way to the H. Is it just because my car is old and this is one of the things that starts to happen, or is there something specifically I should be looking for? thanks
