Why is it still painful for my girlfriend when we have sex?

We've been having sex for 3 months now and she has clearly lost her virginity. And we were on the impression that it only hurts the first few times. However the pain for her is rather unbearable. And we would like to know what may be the reason. And what we could do to help her.
We have tried several different positions.
 
What probably happened is she got a tear or cut inside the vagina from too much sex or sex that is too rough. I would say abstain from sex for 10 days for it to heal because abrasions in the vagina take a longer time to heal because it is an internal injury.
 
maybe she is too dry or you're too rough. you can start off with figering her first. take it easy.
