Why doesn't my GM starter engage to the flywheel?

When I first installed this new(rebuild) starter, it turned the engine just fine. But that was only after one crank. The second crank it sounded like a clunking sound and the flywheel doesn't seem to be turning. Would a shim fix this?

this is a 72' ventura with a chevy 250 engine.
