Why does one of my spam emails contain a lot of my personal information?

Most likely you have previously responded to a spam email (you probably didn't realize it was a spam) and gave them this information.
 
I have a spam email offering a T-mobile Sidekick Slide. It went right to my SPAM box. The email contains my full name and my home address. How did this happen? I'm sure it's spam, and I have not responded. Also, the "sender" of the message is displaying the same last name as mine. I'm worried about seeing so much of my personal information in a spam email. Has this happened to anyone else?
