why does my 1997 toyota camry jerk while im trying to accelerate?

It happens about 2-3 times a week and during rainy days. when i try to accelerate my car jerks then finally shifts gears. Its like my car is hesitating to shift gears
I just recently gave the car a major tune up and the jerking still occurs.
