I mean sure, sometimes they're throw in a couple of completely out-of-place things just to go "see, look, its actually random just like real dreams are" but the dream is still largely coherent. And dreams are not anywhere close to coherent in real life. The only dream sequences I've ever seen on film that were anywhere close to as incoherent as real dreams were in an Italian comedy who's name escapes me at the moment, and even those were mostly just surreal instead of actually being incoherent.
