Who died and how on Ghost Whisperer TV show?

Well her husband died, but he eventually found a new body to occupy, she told him not to do it but he did it anyways at the scene of an accident but then he
didn't remember her for several episodes, when he did they married again and had a child.
(in the mirror you see the other guy but she still sees him like he always appeared so its a little weird at times)
