Where in the USA have CAFO's most significantly impacted the environment.?

The EPA's Region 7 (Iowa, Kansas, Missouri, and Nebraska) has 20% of the countries CAFOs. I couldn't find any specific incidence of environmental impacts, but a list of general impacts are listed on the EPA's website (follow my link), under "How Do CAFOs Impact the Environment".
