want a sports physical? Well I have to get a sports physical because I am trying out for cheerleading and I'm not sure what I'm supposed to do. I called the hospital to set up an appointment and the lady on the phone said that I could bring a form in and the doctor would fill it out for ten dollars. But I don't have a form so I really dont know what to do.If you could help I would very much appreciate it!
When you get a sports physical do you just walk in to the doctors and tell them you
