Anywhere you want. You'll need a 4-year college degree, 4 years of med school, and at least 5 years of residency for that job - and you don't even start to specialize in surgery until 2 years into med school, much less heart surgery (that's your residency). You just need to take the premed classes, and since you can do that pretty much anywhere, pick the best college or university you can get into and go from there.
