She might enjoy a book (even a book on tape), or maybe watch a movie with her, or if she likes music maybe something like a cd, or maybe going somewhere with her if she's able like to a museum or gardens and wheel her around in a wheelchair (if she has one). She might enjoy a photo or photo album of some sort. I bet she would most surely enjoy your company for a while at least.

The last thing she needs is a nick nack or another sweater. Give her an arrangement of fresh flowers. They say I love you, but don't stick around to collect dust. Or take her out to lunch. Or get her a manicure and pedicure.

No matter what age a women is, she is always young!
Women, specially 40+ and all generally like gifts that make them feel young..
You can give her something like a perfume, handbag, dress or a watch?
And ofcourse give her a hug... and a rose.. and dont forget to tell her you love her :D
