What to do if you're a poor test taker?

I have been noticing it's a trend for me to do poorly on tests, but I get excellent grades on assignments and in-class work. I don't fail the tests, but considering that I tend to get A- and higher on assignments, getting a C seems relatively low for a test score. I attend class everyday, I participate in class, and I ask for clarification if I don't understand something. Why do I do poor on tests?

Oftentimes after I get the test back to see how I did I realize that I could have easily gotten the correct answers for the questions I got wrong. I'm able to concentrate and pay attention, but maybe there's something else I need to be doing more in order to get higher test grades. What do you suggest? How should I study? How far in advance should I study? Other than rewriting notes to study (which I hate and doesn't really help me much to be honest), how else can I study to do well on tests? I know for this upcoming school year I want to do better than I did this summer (which wasn't bad) but I know I could have easily earned A's instead of B's. I know, I have high standards!
