What is wrong with my 1994 Honda Accord?

I have a 1994 Honda Accord LX Standard and when i go from 2nd to third gear its starts to jerk this also happens in 4th and happened once while in reverse. It feels like if your in 1st gear and dont give the car enough gas. This dosen't happen all the time but enough that i notice and worry about it.
