What is the title & author of a book about a boy helping his Grandpa to walk again?

It's about a boy helping his Grandpa learn to walk again after he fell ill. The boy describes his mom telling him his grandpa is sick and then when he comes home from the hospital the boy encourages him to walk again. He helps him by putting his arm around him and saying one foot first then the other.
I can recall the cover and the title has something to do about one foot in front of the other or some sort? I have it in my minds eye but just can't grasp it enough. Please help me, this is really bugging me! Thanks so much!
