What is the name of this book? It has lots of riddles, etc in it?

...It starts with a K, and it's like "K(something) on the beach" or something having to do with a beach.

My friend told me about it.

It's about this doctor who kills a cat, and this boy who either runs away from home, or has an abusive family.

They're separate stories, but eventually they become entwined. In an interview the author said you have to read it more than once.

I haven't been able to get in contact with my friend, so I figured I'd ask here.
 
I have not heard of this book, but googling it is a more good idea, since you know more about it, or you can go and ask your local library?
