What is the cost of switching current Verizon phone to iPhone?

Hi, I have had my phone (Samsung Intensity) for almost 2 years, my mom said she marked on her calendar, that I am available for an upgrade in a month or so. I had my iTouch stolen yesterday at the mall, so I wanted to get an iPhone. How much would it cost for:
-the device
-the data plan
-to switch the phone
And please give me the total also. Thank you so much!
P.S. I would want only the 16 GB. And it'sthe 4G, I think.
By 4G I meant the 4th Generation of the iPhone
 
Since your contract is almost ending, when you upgrade your cell phone it would cost you 199.99 for a 16GB iphone 4G model. You will be required to subscribe to a monthly unlimited data plan for your iphone which will be 29.99 per month. The lowest monthly voice plan which gives you 450 minutes will cost you 39.99 per month. Then if you wish to add unlimited texting you will pay a extra 20.00 a month.

So your bill would be the following 39.99+29.99+20.00 = 89.98, Then add the state and fcc taxes which add up your total bill of 92.00 to 93.00 per month for 2 years.
