epidural fat is the the layer of fat surrounding the dura, which is a tube or sheath the spine lies within. ventral means "front."
it seems to indicate the disc between L5-S1 has a centerline bulge (probably mainly rearward but it doesnt state that specifically) that is slightly touching and denting the front surface of the fatty layer around the dura layer the spinal cord is in.
