What is a good exercise plan that I can do at home without any equipment?

I'm 16 years old, 5'8'', and 120 pounds. I want to be healthier since I never exercise, and I want to eat healthier since I always eat junk food. I also want to gain a lot more muscle, since I'm practically skin and bones. What exercises and how many of them should I do to get cardio exercise and to get muscle mass? How often should I do these in a week? How many reps should I do in a day? Also, what should I eat in order to help gain muscle mass and stay healthier? I will really appreciate any great, well-thought out answers for this. Thank you so much in advance!
