What happened? When I breath in it feels like a sharp stabbing under my left shoulder?

I already have mild arthritis in L1 and a bulging disc in between L5 and S1, Anyway, I was sliding down the counter (using it like a rolling pin up and down my back) and all of a sudden I had an extreme sharp pain and that was about 20 hours ago. The pain is more severe when I take in a breath. I have tried adjusting my back, stretching, doing basically everything I could think of to make it feel better.

I have pain medicine and it isn't even helping, the pain is next to my spine but a little more to the left under my shoulder blade.

What kind of injury does this sound like? Could I have made a disc herniate? Is something just out of place? Od does it sound like a pulled muscle?
