What are the three most common motorist caused car-bicycle crashes?

This is in Texas by the way; not links, just answers please.

and another question while I'm at it;
when is a bicyclist not required to ride on the right side of the roadway?
I just need three,
and I said no links.
 
1. Left turning car, rider was hit going straight.(usually at intersections)
2. Right turning car, cyclist hit when trapped between car and curb
3. Car at driveway or intersection fails to yield to bicycle already in intersection.

A bicyclist is not required to be on the right side of a one way roadway when turning left onto another roadway.
