What are legal requirement to start Spa Business in Philippines?

On the Government website in Philippines it doesn't provide the specific requirements for new Businesses.

Can somebody please direct me where I can find the mandatory requirement for new Spa or Massage Center. Is it mandatory also to get a DOH permit before you can start your operation?
