Weird question for a doctor?

Sometimes when me and my husband have sex, his skin tears on the tip of his penis and he starts to bleed. It's happened twice within the last 4 months. Is this something that he needs to get checked out or should we just use lubricant? Thanks for your help.
oh yeah, it didn't start to do this until a year after we were married.
