Weird pinky toenail on right foot?

Need some help -

I don't have the insurance to see a doctor right now. Let me give you the situation - and please, don't reply if you're going to tell me to see a Dr. because I just can't right now.
* I have a thick, flaky nail on the pinky toe of my right foot. It's been there for at least 15-20 years and has never spread. It's not discolored, and I really don't think it's fungus.
* The nail itself is tender and sometimes bleeds - or maybe it's a callous under the nail, but it seems to be the nail.

It's becoming painful and embarrassing. Does anyone know what this might be or how to treat it? I file it down and clip it, but it just comes back the same way.
 
If you've had it for so long, why the sudden desire to know what it is? It's probably just the way your body was made, a deformity that really doesn't interfere with your life.
