Think you've summed it up pretty well there In my opinion David B just hasn't been as good in anything else since Buffy/Angel.

David B's character, hence the killers nose bleed through the mask, and his nose bleed at the end

A lot of people somehow got confused by that.

According to Jamie Blanks (Director) There was going to be a scene where we see Dorothy attacked by the killer and dressed as the Cherub but it was cut.

So in the final version that is why Dorothy is wearing the Cherub disguise because she is attacked off camera.So it is meant to look like to the other girl that Dorothy was the killer all along.

I have to admit the first time i saw this movie i was confused by the ending but it was explained on the Audio Commentary.
