Trying to get used to the strange world of North American sports?

I emigrated to NA over 12 years ago but I still don't understand how NA sports tick. For instance, the batting line up in baseball. I am aware that the #1 batter in the line up is called the lead off batter. However, this is only certain to be true for the 1st inning. Thereafter anybody in the line up might be the lead off batter. Anyone might be the clean up batter. For instance if it's a 1-2-3 first inning then batter #4 now becomes the lead off guy. The designated lead off batter might not be the lead off batter again until the next game! So what is the point of the batting line up? (Also why do they call the final the World Series and the winner the World Champion when they don't even play against teams outside North America?)
Neither answer addresses my main point: the lead off and clean up guys are only lead off and clean up FOR THE FIRST INNING, and as far as the clean up guy is concerned maybe not even that. In the next inning the 4th guy might be the lead off hitter and the 9th guy the clean up. The line up only makes sense once per game so I see no point in it.
