Transfer on death deed? Lawyers please answer!!!?

Hi I am in quite a pickle. It's a very long and complicated story but this is a quick summary: My adopted elderly mother signed a transfer on death deed and listed me and my sister as the sole beneficiaries of her property in 2005. Unfortunately in 2008 her health dramatically declined and she was diagnosed with dementia with alzheimer's and her estranged biological son was awarded her legal guardian. Anyways, I just received a letter in the mail and I have 28 days to respond that he needs the house to pay for her medical care in the nursing home but I basically have to sign all my rights away to the house. (My mother wasn't well off but she managed to pay the entire mortgage off before she retired) Is there anyway to stop him from trying to steal the last of my inheritance???? Or should I let him sell the house to pay for her nursing home care??? Is there ANY other way around this????

P.S. I wanted to keep my mom from going to a nursing home but I was going to school and working and I'm only 26 years old! I was recieving no help and the only advise I was given was from the county was that she was too old for me to take care of and she needed to be put in a nursing home which crushed me and I couldn't do it so her son stepped in to finish the process but he is clearly doing it for the money!
