I'm filling out a Trial by Written Declaration Form for California (http://www.courtinfo.ca.gov/forms/fillable/tr205.pdf) and there is a part on the first page which states "TO BE FILLED OUT BY COURT CLERK" and it doesn't necessarily specify which portion. The instructions (http://www.courtinfo.ca.gov/forms/documents/tr200.pdf) do not specify either. I know these things can be thrown out on the smallest technicalities and therefore, of sections A, B, C, D and E, I need to know which part is it actually referring to as what shall be filled by the clerk.
