I'm positive it was never mentioned. But as I said, it appears Kirk spenRAB most of this film as NOT the Captain of the Enterprise anyway.
I'm actually betting that he won't get his Captain stripes until the very end of the movie. In the trailer there is only one shot of him wearing his yellow shirt and he says "buckle up." I reckon that's the very end of the film (trailers always seem to stick a scene from the very end in somewhere!) as Kirk finally takes command.
To be honest, they could quite easily play with that sort of continuity without screwing things over. If it is mentioned at all, it is no more than a throwaway line (and I'm still convinced its never been mentioned outside of books) and Trek has a history of small continuity fluRAB that matter very little. Besides, we could just put it down to Kirk and Carol being so old and wrinkly by St2 and 3, that their memories had got a little muddled and she actually helped Kirk celebrate his promotion.
As for Riker and the holosuite, only two men are capable of pulling off something that trite! (also, note your unforgivable continuity gaffe - the Enterrpise-D had holoDECKS. Tut tut.)
