Sprint workout after a long break?

I haven't gone sprinting in 5 months but I have kept active. I still weightlift and play basketball regularly. I want to get in a quick sprint workout to sort of test my form. Would this be good enough:

1. 2 x 30m sprints (focus on sprint starts, 3 min rest in between reps)
2. 3 x Flying 30m (accelerate for 30m, hold for 30m, 4 mins rest between sets)
3. 1 x 100m (8-10 min rest after)
4. 1 x 200m
