sprint rebate question?

so i got a sprint phone with discount with my employer (kroger co) but theres 2 rebates. one is a regular and the other is EPRP (Everything Plus Referral Program) mail-in rebate form. which one is the correct one for my plan. in the referral program is for customer who are getting a discount. and i am but not through a employee refrerral but through my employer and company i work for so which is the correct form. 10 pints
