Sports conditioning program?

I play lacrosse and the season starts in about 6 months (I like to be prepared!). This year, I want to make varsity, which I think I have good chances of if I get in shape. The problem is, I don't know where to start. Does anyone have any conditioning schedules or steps to make a schedule? Mile time doesn't matter, and I can run 1-1.5 miles now.
