Spin disc extrusion , need advice!

I am 57 years old. I felt severe pain from my right hip running into right leg about one and half month ago. My right calf also feel pain and part of right toes feel nurabness. I did MRI , the impression:

There is a large,( 1.7cmx1.7cmx1.1) , superiorly directed, disc extrusion at the L5-S1 level other less severe degenerative changes are present as discussed above.

I have used some pain and anti-inflammation medicines and injection to treat for one month , my symptoms already calmed down about 60% but the rest 40% is stably and no longer change. I can’t sit too long and right leg can’t walk too long. My doctor is a pain management doctor to treat sports related problem, Her point is that the disc extrusion compresses my sciatica nerve and leg nerve. She think it can’t be healed without surgery and suggested me to see a Orthopedics or Neurological to prepare surgery. I asked her if I can try stretching exercises or physical therapy, she said these treatments will not help because the extrusion is too big to be pulled in and maybe stretching exercises may make worse if the extrusion is broken.

I would like to know if there is a deadline that I have to make decision to take surgery or not.I saw some material said it is six months. I will try any other methoRAB before the “deadline”. Any advice will be appreciated.

I already saw one Orthopedic Surgeon and one Sugery Neurological. They all suggested me to take Surgery but still let me try other way. I asked them when I have to make decision.They said as long as I don
 
Are they planning on removing the the spinney process by doing a laminectomy or did they give you another name?
 
The surgery is the same risks as with most surgeries. It may help you and it may not, and it could possibly make you worse than you are right now. I would ask your doctor what the odRAB of it resolving your pain and where, such as your back pain, your sciatica, etc. This is still not guarantee mind you, but it will give you a better idea on what you are looking at.

As to a timeframe.. there is no set timeframe for anyone. We are all different and your situation is unique to you and your body. I can tell you the longer that a nerve is pinched, the greater risk for permanent nerve damage.

No, the Orthopedics just told me he is going to do a microsurgery , it will cut
me about one inch and take anout 45 minutes to one hour.
the another Neroulogical said he will do Laninectomy.but he said maybe cut
big than one inch. I find doctor doen't like patient to know too much about medical .

It sounRAB like they are removing the extrusion and decompressing the nerve. I would try anything to get this nerve to relax before moving forward with surgery. Have they tried physical therapy, nerve blocks, or epidural injections?
 
I did three time Nerve block and trigger point injections, but now my primary doctor lets me try epidural injection. he didn't let me try physical therapy because he thinks it will make worse . He said maybe it will break the extrusion.
