Sin City Question (Contains Spoilers)

I have just finished watching Sin City and think its a great film however one part has me confused.

At the beginning we see an unknown guy with an unknown woman on a balcony who he declares love for and kills her. My first questions are who is this guy, who is the woman and how does he/she link into the story?

At the end of the film we see the same guy in a doctors disguise in the lift where he offers the young girl from Old Town a cigarette while she is on the phone to her mum? Im sure im missing part of the plot here but how does it all link together?

Sorry if im being stupid. If you could help me answer this it will be the final piece of the jigsaw in the film.

I presumed he was an assassin-y type character who was hired by the first woman to kill her (kind of assisted suicide) as she didn't want to go on living (obviously) - presume the declaration of love was a tailored death? what she wanted to hear before she went .
Similarly the lass he offers the fag to in the lift - again I guess she wanted to off herself as an alternative to the certain (no doubt gruesome) death she would have suffered at the hand of the girls of old town.

Josh Harnett's Characters is just refered to as The Guy, but yes he is an assassin who trademark is to offer a girl a cig b4 he kills them.
The First one was an arranged suicide, but no1 really knows who hired him to kill becky.
