sharp pain in nipple during 3rd trimester?

my wife is 29 weeks pregnant and is experiencing a great deal of sharp pain in her right breast, specifically her nipple. i suspect its to do with th immiment lactation but she's in a lot of pain. sort of a stabbing/sharp pain.

any ideas?
