Setup PHP form from Start to End.?

Can anyone show me how to generate a simple php form, consist of Name, Email and Comment box. Also, how do you design Form.PHP?
Previously, I enter some copy and paste of the php but it didn't work because it seems that my web server is missing something...maybe like form.php.
 
the copy and paste should work as long as the site host you are copying the html page to allows php and php is enabled on the server
