Reg 1 pps number Ireland?

Hey,

I'm French, i'm living in ireland, i would like to get a pps number, i checked online what i need to have, and they talk about a REG 1 that i'll have to fill in. I read they will test the level of english, do you do know which kind of question they ask? I'm a bit scared i can't answer, and they will check if i can answer without the help of a 3rd part...

So my question is do you know what there is in the REG 1 ?
