Questions in quotes syntax ( Authors / Writers )?

Okay, so I ALWAYS get these mixed up and I usually just use different wording to make it "work" without having to have a question mark at the end of the quote... Can anyone tell me which of these is actually "accurate"??

A) "Who's there?" she asked.
B) "Who's there," she asked?
C) "Who's there," she asked.
D) ((something else))

Any help?

(I'm working on one of my books & I figured it'd be nice to know what the proper way to do it is, lol)
Yeah, usually I'd just make it like this:

Then she asked, "who's there?"

Just so I don't have to deal with that particular syntax rule, lol
