Question about what a "sex offender" is?

My friend and I were in a deep conversation when he confessed to me that he was listed as a sex offender.
Of course, I was curious to know as to how he was a registered sex offender, so I asked him how it happened.
He told me that when he was twelve, his former step sister, he, and her friend were playing a game of truth and dare. He then told me that his step sister's friend dared him to "go down" on his step sister.
The girls were supposedly fifteen; he was twelve.
He then told me that it eventually came out in school and he got into serious trouble for it. He had to go to treatment and is supposed to be labeled a sex offender for ten years.

I find this very sketchy. How does something like that stay on a minors record? How does he get in trouble for that when he was twelve and the girls were fifteen? How didn't they receive any counseling?

I feel like he might be hiding the real reason why he is a sex offender...but I'm not sure. I didn't even find his name listed in the sex offender registry.

I live in MN. If anyone can help explain how someone becomes a sex offender, I'd greatly appreciate it.
