PUK and PIN help!

I tried to turn on the PIN code system thingy but then after 3 times of incorrect PIN,the phone asked me for PUK code and I've been trying and then I got it.So now it's asking for a new PIN code so I put em and then it asked me to verify the new PIN code.I tried putting the new PIN code again but it says code error...? I've tried a lot of times but it is still like that.Help me please! I just hope that it isn't broken or anything.:(

And now...My Sim card has been rejected...>.< *sob* *sob*
