Practice vs. Actual Swing Divots Using Irons?

During every iron shot I always take a practice swing. Over the past few rounds, I've noticed that my practice swings usually take a nice small size divot in line with where the ball is (almost feels like it would be a perfect shot had it been the actual shot). When I address the ball and swing away after my practice swing, I typically end up chunking the shot creating a crater sized divot which undoubtedly leads to China. This clearly results in a shot 40-50% shorter than my typical Iron shot usually skimming the ground or barely getting up in the air. I haven't noticed anything else in my play (I'm an average player with about a 28 Handicap).

What am I doing wrong? Is this jitters? No confidence in my actual swing? What could it be?
 
1st) You probably are making a "swing" on the practice, then more so "hitting" the ball. Just make your swing and let the ball get in the way.

2nd) Most likely you are moving your head when actually swinging at the ball. Concentrate on keeping your head still. Moving your head is normally the culprit when hitting the ball fat.
 
I'd only practice swing for shots that require something less than a "full swing" like putting or a close approach shot onto the green. So I'd say if your practice swing is always good then by all means use it!

Don't stress too much. Forget about trying so hard to hit the ball. Just relax, swing and the ball is going to fly.
Your confidence gets afected when you approach that little white sphere. So the best is, do it like you know, address the ball the right way, and swing like the practice and it is going to be fine.
