PLEASE!!!! My rice sometimes comes out perfectly fluffy and cooked, sometimes....?

just mushy on the outside and crunchy(not all the way cooked) on the inside! how does this happen!?? what am i doing wrong? there was enough water and more than enough time... 1hour or more! and still not all the way cooked! I browned it first, then added the water(2x the rice) brought to a boil, lowered to a simmer and covered simmering for over 20 min
for spanish rice, browning adds flavor its like whay tou do with rice pilaf
sorry for that spelling... anyway I am not looking for directions, if you notice I wrote how I did it and it is exactly what the next comment said .. I just want the science of it all...maybe the SIMMER was not hot enough or too hot??
